Why appointments are missed
The causes overlap from one profession to another. The first is forgetting, all the more likely when the appointment was booked a long time in advance. Next come last-minute problems, a sick child or a delay at work, which the person does not report because they do not have time to call or cannot get through to anyone on the phone.
There are also appointments that have become unnecessary: the symptom has gone, the customer has found another tradesperson available sooner, the need has changed. If cancelling means calling during limited hours and reaching voicemail, some people give up on letting you know and simply do not turn up.
What an empty slot costs
For a healthcare practice, a missed appointment is a slot that could have been used for another patient, at a time when waiting times can be long. In France, the practitioner cannot recover this loss financially: according to the MACSF, a French mutual insurer for healthcare professionals, healthcare professionals working under the national health insurance agreement (conventionnés) may only charge fees for care actually provided. A penalty for missed appointments set out in France’s Social Security Financing Act for 2025 was struck down by the Conseil constitutionnel (France’s Constitutional Council) on 28 February 2025, not on principle but because its nature, amount and implementing conditions were not sufficiently defined.
For a tradesperson, the cost takes another form: a wasted journey, a half-day reorganised, a job postponed. For a hair or beauty salon, it is an hour of unbilled work. In every case, the lost slot cannot be made up, unless someone can take it in time.
Lever no. 1: confirm the day before
A reminder the day before or two days before targets both forgetting and last-minute problems, bringing them to light early enough to reallocate the slot. In its advice to practitioners, the MACSF mentions sending a reminder SMS or email a few days before the consultation and calling the patient the day before the appointment.
A confirmation call has one advantage over a written message: it allows an immediate answer. If the person says they cannot come, the slot is freed up on the spot and a new appointment can be offered in the same conversation. To avoid tying up your team, these calls can be grouped together at a quiet time of day or handed over to a tool.
Lever no. 2: make cancelling easy
Making cancellation easier may seem counterproductive, yet it is what turns an outright no-show into a slot that can be reallocated. A person who can let you know at any time, in a minute, no longer has any practical reason not to. State clearly in the appointment confirmation how to cancel or reschedule: an online booking link, a number that answers outside opening hours.
Word the request kindly, too. A message along the lines of “if you can’t make it, please let us know so we can offer the slot to someone else” gives a concrete reason to do so, without making people feel guilty.
Lever no. 3: keep a waiting list
A slot freed up the day before is no use if nobody is ready to take it. Keep a list of people who would like an earlier appointment or who could not find a slot, along with their availability. As soon as a cancellation comes in, you know whom to call.
For repeat cases, dialogue remains the first response. The MACSF points out that in medicine, the French code of medical ethics allows a practitioner to stop treating a patient under certain conditions, notably by informing them and ensuring continuity of care, but this is not a decision to be taken lightly.
Confirmation calls taken care of
Alyas answers your calls 24 hours a day based on the information you provide and, once connected to Google Calendar or Cal.com, can book appointments over the phone. Outbound service calls, such as appointment confirmations, are set up as part of an assisted onboarding. Each call is transcribed and summarised in your customer area. The ten-day free trial lets you judge the effect on your diary.
Frequently asked questions
Can a doctor charge for a missed appointment?
In France, according to the MACSF, healthcare professionals working under the national health insurance agreement may only charge fees for care actually provided. The penalty set out in the Social Security Financing Act for 2025 was struck down by the Conseil constitutionnel on 28 February 2025. Check the state of the law at the time you are asking, as the subject is regularly debated.
When is the best time to confirm an appointment?
The day before or two days before: late enough that the person does not forget, early enough to reallocate the slot if they cancel. The MACSF mentions, among other things, calling the patient the day before the appointment.
How can a slot freed up at the last minute be reallocated?
By keeping a waiting list with everyone’s availability and, as soon as a cancellation comes in, calling the people who wanted an earlier appointment.
